Itching to tell someone

Everyone knows it is rude to discuss medical conditions in polite company. But sometimes the solution to a lifelong mystery falls into your lap, and you just have to let people know.

As a kid, I never understood why nearly every place we went on vacation, the souvenir shops all sold back-scratchers. Think about it for a second, here is an item so universally available as to suggest that there are a lot of people out there with itchy backs. Yet to a kid, this seems like the dumbest product in the world! If once-in-a-while you get an itch in a plce you can't reach...you just ask a friend or family member to give the old back a scratch. Simple right? Whoever invented that back-scratcher was some dummy!!!

It wasn’t until my mid 20’s, when I started to notice an itch on the same patch of my right shoulder blade, that those souvenir back-scratchers began to make sense.

For most of my adult life I have suffered silently with this annoying back itch, and I’ve spent a lot of time acting like a big shaggy bear, rubbing against doors, coat hooks and trees. I’ve even enlisted friends, family, and coworkers to assist me in reaching that annoying spot. One of my assistants finally got so sick of me asking that she bought me a Goofy back-scratcher while she was on vacation at Disney World (thanks Sara). I even have a friend who has taught his dog, Casey – a big English Pointer mix – to scratch his back on command! Clearly this is a widespread phenomenon, yet people don’t really talk about it much.

I visited doctors…tried anti-itch creams and moisturizers…nothing helped.

Then one day the little light bulb in my head began flashing as a fellow musician began telling me about an itch he had had on his right shoulder blade for most of his adult life. Unlike me, he had found a doctor that was familiar with the problem and was even able to give it a name: notalgia paraesthetica.

Very simply, this common condition occurs because the nerves that provide sensation to the upper back are very susceptible to damage at the place where they emerge from the spine. It is theorized that heavy lifting, or some other sort of back trauma is somehow to blame, so ironically the place where one feels the itch has nothing to do with the cause. This is what fancy-pants doctors call a 'referred sensation'. So, short of deadening the nerve endings (with something like Capsaicin) or doing a nerve block at the source, there really isn’t much anyone can do for the problem.

The good news is that I now understand that I am not alone…far from it, in fact. All those back-scratchers in all the souvenir shops around the world were made with me in mind (along with all the millions of others walking around with this itchy problem).

Somehow, knowing the cause, and the name, of my itch is important. It doesn’t make it any better (or worse), but it allows me the illusion of control over my health…something that becomes more and more imperative as my lifeboat drifts dangerously close to the shoals of middle-age.

Strange journal entry today? Yes.

Helpful? Only you can be the judge of that.

Reason I wanted to share? I’ve been itching to tell someone.

I found this article after researching what the doc told me was wrong with my back after 18 years of "suffering in silence" like you! I am so glad I found someone that has the same thing. Is your back a scarred up, discolored mess also around your scapula?? UGH! I still think it's nerve damage from an epidural I had with my 10 pound son nearly 18 years ago. I have anesthetic cream now...it didn't work for you?
Thanks for sharing it!

I too have had these annoying itch on my left shoulder blade for quite a few years now. It has gotten an ugly color on my skin. I feel so much better too just knowing that the condition has a name. Aside from getting rid of the itch I would also like to know if someone knows of how to get rid of the "mark" it leaves on the skin.

What would we do without Google!
I too suffer with this stupid itch.
I am glad you were itching to tell because I was itching to know.

Hello!

I've had the same itch on my right shoulder blade ever since i was a child. The really weird thing is when i scratch it, i get an itch below my right nipple, directly opposite where the itch on my shoulder blade is!! I don't really get it that often, but its always been there. I kind of enjoy the relief of scratching them both though, so it doesn't really bother me! Hope that doesn't sound weird!!

Thanks for "kind of" easing my mind....I also have this 4 by 7 inch area on my upper left side back that is my "itchy spot"...I have given my kids a dollar to scratch it! Sometimes though it is almost like tingly, "asleep" but mainly just itchy...I ALSO thought it definately started after my first born in 98 and had an epideral...I brought it up to my OBGYN and he seemed to "shoo-shoo" it as rediculous....lawsuit paranoid maybe???but you sign your life away before the epideral anyway when in major pain so dont really care what the heck you are signing...just give me the needle!! Plus I had bad back labor with my first as well before/after it wore off...I just keep thinking skin cancer

Isnt it amazing what a google search can do. Well thats reassuring. Its been driving me mad!! So its sort of quite normal! Not that I am.

I had a car accident almost 3 yrs ago and have had thoracic pain since in the t-2-t-6 area ,I rubb the bones there constantly ,I thought this was what discolored the skin and then thought my rubbing caused an itchy infection rash ..I now see it is nerve damage diagnosed by neurologist, what none of you state is any pain though..the bones in the area of my back feel like live nerves ,when streching this area ,like to move you arms i get an extreme pain almost like a charlie horse there ,the more pain the more itch , the skin that is discolored is numb ,but the pain is deep into the bone ,anyone have extreme pain like this ??

I have been itching for at least the past 15 years. My spot is exactly where the back of my bra hits and has cost me a fortune in bras - trying to figure out what was sticking into my back! Finally I just stopped wearing bras - on a double D, that's not an easy decicision, but the itch was making me insane.) Now that I know it's not the bra (thankfully I can once again give my breasts the support they need!) I am trying all of the remedies - capsaesin just seemed to divert attention to the burn from the itch. What types of topical anesthetics are you trying?

Posted by: Lacy Notek | Sep 5, 2006 7:51:20 PM

This is such a relief; not to the itch, mind you ! I was told the name of the condition a week ago & given a script for ELOCON OINTMENT 0.1% to apply to the area (on back below right shoulder blade)of which I have yet to acquire & start using. I too, have been a victim of this ridiculous condition for over ten years (now aged 51)and have a pale light brown patch about 4cm x 5cm. I guess I am somewhat pleased to discover that there are others with the same, which is kind of bizarre!!

Posted by: Leanne D. | Sep 6, 2006 1:29:25 PM

I have been suffering with this for about 10 years and I am 50 now. It is really horrible to constantly feel like if you don't scratch you will go insane. And scratching is just a fleeting relief action. The itch is so deep it just resurfaces fast. Also, my skin is so tired and sensitive that I would not even call it relief. Capsacian was a joke. Oddly, I take 5mg Ambian at night and I get some really noticible relief, but is that a long term solution? What about epidural? Is it worth it?

Posted by: NJ Fedder | Oct 4, 2006 4:17:31 AM

I have this same type of itch on my left shoulder blade. I also have terrible pain in my neck like a burning sensation. Also I have had an epedural like many others have mentioned they had too. I buy an ointment called Foil online and it's got something in it that numbs the itch, otherwise it would have driven me crazy. I've had this itch for at least 7 years or so, if I remember right. I'm nearly 50 too as somebody else said they were. I don't know if that has anything to do with it, but it is good to know that a doctor has given this itching a name.

Posted by: Karen | Jun 13, 2007 12:59:34 PM

I'm in my fifties and have the same itchy spot right where my bra goes across my back. I was wondering if it's from a mix of perspiration and maybe laytex/spandex or something in the elastic rubbing the area for a long time. My job demanded wearing a balistics vest, so I had a lot of sweaty bra time. I have no pain associated with the numb spot - just a recurrent itch that makes me crazy because the numbness blocks out some of the benefits of scratching.

Posted by: Annette | Aug 22, 2007 6:31:27 AM

Yes, thank God for Google. I googled "reaccuring itch in same place" and it brought me here. My patch is right in the same place as the lady who thought it might be something in the bra strap or metal clasp. Then why would it not be on the other side as well. Mine is on the right
side just under the bra strap, close to the metal clasp. Makes no difference if I wear a sports bra either as I've tried it all. Even bra less it itches as soon as the t shirt touches it. I have found that it flares up really bad sometimes and drives me nuts. If I don't drink a ton of water it gets worse.

Posted by: sherry | Aug 29, 2007 8:22:35 AM

I am 52 and have the same itch on my back just above the bra closure, left side. I have had it for at least 7 years and it makes me miserable. I am now using a lidocaine patch but I get a reaction to the tape and have to take breaks from the patch use.I heard that some people get injected with Botox to paralyze the nerve- have you heard of any success with this injection. Also I fear I may be getting an itchy spot on my lower back. Does anyone have the itch in more than one spot?

Posted by: Judi | Nov 1, 2007 3:36:18 AM

I have had the same annoying painful, itchy, and numb sensation below both shoulder blades, and sometimes in the lower back area for the past 6-7 years. It seems to drive be crazy if I don't insert a pointy object deep in the area, or massage deeply with a pointy object. Along with the relief comes brief sharp pain. It is so strange. Numbness, pain and itch all in one spot at the same time. I am 51.

Posted by: sandra | Nov 1, 2007 5:03:37 AM

I have a spot that is numb and when pricked with something sharp, it feels like I got stab, very painful. Yes, it also itches and at times gets dry.

I seen a dermatologist and he refered me to a neurologist and a biopsy. Since then I have now developed a second spot on the left side of my mid back, close to the spine. Exactly opposite of the fist spot.

I just want to know what it is and make it go away.

Someone please help with some answers.

Feel free to email me regarding this matter if you suffer the same as me.

Thanks for all the info everyone! I am 35 and have had the notalgia paraesthetica diagnosis on my itchy discolored spot by my left shoulder blade. It has been there for about 7 years. About a year and a half ago I started itching on my breasts (someone mentioned something like this above). It is a deep, insatiable, rather crazy-making itch! My docs now are having me do an MRI to rule out more serious conditions. I would be curious if anyone else has had a the spread to the front of the body?

Posted by: Lisa B | Mar 22, 2008 4:23:36 AM

Just checking in to see if anyone has found a successful treatment. I last visited this site in Nov 2007. I have received no real relief despite seeing doctors. I tried acupuncture which did relieve the intensity of the itch but it has not gone away. It has not spread to another area. If anyone has a recommendation please let me know

Posted by: Judi | May 10, 2008 2:16:05 AM

when I started hormone treatment I had an itchy spot on my neck which would not go away. Cold compresses, sleepless nights the whole nine yards. After several visits to the dermatologist I was started on a low dosage steroid cream which seemed to clear up the problem. Whenever I had a flareup I would use the cream and keep it under control. Several years later I had a severe sunburn on my upper arms and the itch came back with a vengence,
non-stop. The steroid cream would not work. The itch was like bee stings only itching. Hard to describe. I used the cold compresses and every over-the-counter itch medication I could get my hands on. Nothing worked. I could not sleep and acutally was halucinating from sleep deprevation. Our local small town doctor prescribed Doxepin to help me sleep and Doxepin in a glaxal base to apply to my arms. I did not have a rash or broken skin. This combination worked like a charm. Now I am not taking the Doxepin to help me sleep but every now and then I will get a flareup of the itching on my upper arms and with one application it will disappear. Thank god. Had to share this after watching a documentary about of number of people suffering from skin itching problems. Hope it helps.

I experience almost the same problem (numbness & itchiness) on the right side of my back for quite some time. So thanks for the info.

First...pardon my english...i am from sweden, but i will do my best. I have had an itch on my back for more than 10 years but it is recently I learnd that it had a name a diagnose...NOTALGIA PARAESTHETICA. I,ve tryed capsaisina, subcutane painkillers, cortisone, but nothing helped. Now I am going to try subcutane BOTOX, i HOPE it will help or I am going CRAZY...I will let you know...SOON

3 hours since my botoxshot on my itchie bitchie spot...so far so good!!!

Posted by: Cecilia | Nov 10, 2008 6:15:18 PM

I have 2 numb itchy spots on my back, one on top more near my neck and one on my mid left back area. The skin is discolored and when the itch comes I can literally dig with a scissors and because the surface is actually numb I do not feel that I am creating small open areas cause it feels like I have to dig to get relief. The itch continues until the scabs slowly heal, then I am good for about 2 weeks and the vicious cycle starts over again.

Posted by: lori | Nov 16, 2008 5:41:40 AM

two weeks since my botoxshot, sorry to say it did not work as good as i hoped! the itch was gone for about 3-4 days but now it is back :( i only got 1 ml of botox, so next time i will try 2 ml instead, i hope and belive it will work, i am not going to give up!

Posted by: Cecilia | Nov 21, 2008 12:27:54 AM

Ceclia, for God's sake stop digging yourself with scissors. It seems apparent that while the Botox treatment you received and worked for a while was also the cause of numbing of your back to the extent that you don't even feel the damage you're doing by digging with scissors! Scratch, if you have to, but scratch with nails! I started looking at this sight b/c of "bra burn" which I believe is latex related. I'm in pain and great discomfort but trying to rule out factors and find remedies - trying a prescriptive topical ointment along with antihistamine. Maybe more will be revealed with time. Watch this space ...

I thought I was alone in this....I itch my back constantly...bra height left side. Went to new doctor and he was shocked at the huge brown spot on my back from this constant itching. My skin has thickened and gotten dark from my constant attack on my back. Now it is spreading to the top of my left shoulder. Latest idea from new doctor is contact dermatitis from my bra. I now am using Triamicinolone Acetonide OIntment. Soothing but I still itch. I will tell him about all the experience of the above comments. Thanks.

I too have this crazy itch on the right side of my back near the bra hook area. For a long time I thought it was the bra hooks, but after I stopped wearing a bra, the itch was still there. I was getting worried about it, but since finding this website I am somewhat comforted. However, I am wondering if the itch is a symtom of something else that may be going on for all of us.....My itch comes and goes, but it there everyday.

I found this description of NP online that I thought other itch sufferers might find interesting:

Notalgia paresthetica (NP) is a common, refractory sensory neuropathic syndrome with the hallmark symptom of localized pruritus of the unilateral infrascapula. It is generally a chronic, non-curable condition with periodic remissions and exacerbations. While the dermatologic syndrome may be multi-factorial in etiology, its possible association with underlying cervical spine disease needs to be evaluated for proper treatment. Radiographic studies of the spine may be more considered than they are currently. Collaborative multi-specialty evaluation by dermatology, radiology, neurology, and orthopedics may be indicated in primary management of this condition. First line therapy for notalgia paresthetica with associated cervical disease may include non-dermatologic spinal treatments such as spinal manipulation, physical therapy, massage, cervical traction, cervical muscle strengthening, and oral non-steroidal anti-inflammatory medications and muscle relaxants. NP may in fact be a dermatologic sign of an underlying systemic disease.

If you look for an answer, you will find it, even if it is the wrong one, that is the drawback sometimes with the internet. . . the global library.

Over the last 10 years, I have had an upperback itch that is so irritating. I convinced myself that it was cancer and knew there was a growth on my back. I couldn't see my back, so therefore it must be there.

Thing is, in my early twenties, I hit the side of a gravel pit and 15mph whilst water-skiing and in the mayhem, dislocated my ankle...that's got to hurt..and yes it did. I drove home, as long as my toes were extended the ankle stayed in its socket, all seemed well. Anyway, years later I had an x-ray for suspected colon cancer, turned out it was constipation, but the Consultant did ask, when did you have a rib removed and when was it exactly you broke your back?

I said, I never broke my back and never had an op, he summised, after we conferred that it was the skiing incident 15 years before and that I was born with a rib missing.

So. . . was it a missing rib, or the back trauma that causes the itch? Or just my state of mind?

Who knows, but after a quick scratch I am off similing at the world.

So. . . Morale or the story?

He who scratches. . . smiles! :)

Ahhhhhh, I too have this terrible condition under my right shoulder blade (8-9 years now). With the itch is a sharp stabbing pain, sometimes if you touch (or hit) the middle of my back it could send me through the roof. But most of the sharp pain and itching is in that area under my shoulder blade. I too have the discolored skin there, very odd. Happy to know what it is, but we must find a cure or some relief... can't imagine living with this for the next 50+ years. Someone help us!!!

i too have a rather large dark, itchy patch on my back (left middle). Along with this i hav back pains and joint aches and stiffness. This all after an epidural. Im not alone in this? Thank God!

im a male, 55--about 2 months ago i thought i had a bug or something crawling around my back near the shoulder blade--so i took off my shirt and checked for a bug/gnat/ whatever-well, it wasnt in my shirt-im pretty sure i have this notalgia parsthetica-its real noticeable when i sit at the computer but i get immediate complete relief when i lie down-its a pulling, crawling sensation-i sometimes forget i have it as it all depends on where im sitting (doesnt bother me when im driving) or lying down etc--dont have any problems sleeping i dont think this will be going away anytime soon--crazy! feel free to email me at [email protected] cause i probably wont be on this chat room anymore

Have had this condition for over a year on my upper right shoulder. The itch is agonizing. Have tried Capasazen (?) with some relief. Dermatologist diagnosed this as Lichen Schlerosis which is incorrect I think. Nothing could be done I was told. I also am desperate for relief. I had a series of epidurals back in 1993...you think there is any connection?

Posted by: Barbara | Jun 1, 2009 5:15:37 AM

Thank goodness that I am not the only one! I've had this itching and numbness sensation on my left shoulder blade for a few years off an on. Lately it's been acting up and driving me crazy!Glad to know that there is an actual name for this and that it is NOT cancer, as many of us, it seems thought that it could possibly be. Thanks for this info!!! It really helped!

Posted by: maria | Jun 25, 2009 4:12:37 PM

Yeah omg I thought it was a cancer or something growing too, so ichy and numb... It's horrible! I want to cut that peice of skin off me but it's on my back where I can hardly even
reach or scratch it..!! But then again it does feel good when I do get to scratch it, man thanks so much so the info cause I was worried for a long time, love you Google more then my girlfriend

Posted by: Dave | Jun 27, 2009 1:34:39 PM

I am almost certain that my itching under my shoulder blade is caused by years of taking the pain med hydrocodone that I take occasionnaly for severe abdominal pain. If I don't take it, the itch, which is scratchable, slowly subsides to normal but if I do, then it turns into a burning itch that gradually goes numb and I can't get relief from scratching. I take a spoon of honey & flaxseed oil for relief. Not sure if the relief is a placebo effect or not. I also cut down on coffee, aspirin and ibuprofen which are all ototoxic to the ears as well and seem to contribute somewhat to the nerve itch. I say try detoxing the body by fasting and see if you don't get good results.

Posted by: Itchy | Jun 30, 2009 7:13:06 PM

Wow, another one chiming in. Itching in mid-upper back, right side, from where the bra strap goes to just under the shoulder blade. I think it's from a fall I had in the shower last year, which bruised several ribs and hurt like hell for weeks (but I fell on the other side, go figure.

I'll ask my doctor about it next time I go for a physical.

Posted by: Thera | Nov 12, 2009 4:24:49 AM

I have have had this itchy thing for years off and on - flares up, maybe with stress or unusual physical activity ? I wonder if it is associated with being right or left handed? I blamed my bra, then thought spinal tumour - I'm so relieved to be able to give it a name. My mother has it also with brown skin discolouration - my man tells me there is nothing there when I get him to scratch it, but it is not a huge problem for me so the occasional scratching has not damaged the skin - I'm 53, Mum is 80.

I started looking for information on this topic when a friend noticed a large dark patch between my shoulder blades. She thought I'd been in an accident of some sort, and of course it wasn't something I would notice on my own. I am glad to find that I am not the only one and I suspect that the countless years of scratching with anything handy has a lot to do with the discoloration. I do know that slathering on moisturizer does not seem to help at all.

I have a question for those that feel they may be having a reaction to latex in a bra. I know that I am allergic to some bandages such as elastoplast, and have had reactions to the glue in others. Has anyone else experienced something similar?

I had hade an itch a;so on the right of my shoulder blade for about three years now my docter tells me its the bra strap and that i will have to massage cream for the rest of my life but made to no avail. I was diognosed Diabetes a year ago and after a sweet tooth all my 69 yrs i stopped all sugers my itching stopped and i was happy but as one does from time to time fall out of line having the odd cream cake and lots more ?? and hey the itch is back again on the right side of shoulder. this has happend before so im thinking that this time once again i will be good and tick to NO MORE SUGER.

Soooo glad to have come accross other sufferers of the dreaded itch! There seems to be a common thread here: I'm in my 50s, recently had an epidural...my patch is white; I also have vitiligo..
I love Google dr!

So many of us ! I was given the dx of PN by a dermatologist also about 10 years ago ...he pretty much said nothing I can do for you - its a spine/nerve issue and said google it ! Really he did !
since then I have asked every other doc I have and they all 'SHRUG' their shoulders and say '''um wonder what that is ''' ...then we move on

But ...I have had it for about 20 years at least - it started as a little spot that itched like crazy - I also had the tingling at times ...however mine now has spread (also in the bra strap area) but from one end to the other ; all across my back... and (after a car accident ) about 3 years ago I have it starting in my neck area. I still don't understand it .. strange though because I am also in my 50's ... and I have 3 kids and had an epidural with my now 25 y/o ...weird

To all of you who are suffering with mid back hyperpigmentation and itching....find a good chinese medicine doctor. this is a condition that can possibly be helped with herbal therapy and acupuncture!

Good luck!

Can not believe that I stumbled upon this blog...but to my good fortune today, have learned there is a real "name" for this itch! And the numbers of comments entered, all with the same problem is unreal. To think so many of us had no idea at all what the cause was or how to make the itch stop!! I am 65 and have had this itch for 10+ years now, and on the left side of back in the bra strap area. The itchy area is darken considerably and itches all the time. Sometimes more than other times....but always with intensity. I too have a pair of scissors I use while at work sitting at my desk...and have inadvertantly caused scratch marks while doing so. What bothers me is that in some cases above the itchy areas have spread to other areas on the back!! Thats just great....not bad enough with one insanely itchy area! I guess I would have expected that somewhere along the way someone has found something that would make the itch stop!! I will keep checking this blog.

And I thought I was the only one! Or possibly imagining it... Left side of back around bra strap area - particularly bad at night... I keep a pasta server to hand. Great to have some info and even a name for the condition. Thank you and good luck everyone!
